Application form for first time applicant

Important notes:

This showcase is open to dancers aged 40 and above.
Each performance should not be longer than 3 minutes.
After submitting the application, for any modification, please submit your request to the committee by May 31st. Any modification request post May 31st will not be accepted.
The application will close as soon as the spots are filled.
Please include a link to a video of your past performance at the end of the form, and kindly note that your application is considered incomplete without the video submission.
Application Decision Date: We will notify you on the application decision not later than June 15, 2026
Once your application is accepted, we will inform you how to pay the application fee.
When submitting your application, please submit your answers in both English and Chinese.
